Select Adjust->Pitch Bend Sequence... from menu. The dialog should open.

Select Edit -> Insert Pitch Bend Sequence... from menu. The dialog should open.

In version 11.06, select Edit -> Insert Pitch Bend Sequence to get the dialog.
